ECRI's #1 health tech hazard of 2026: AI chatbot misuse

Every year, ECRI publishes its Top 10 Health Technology Hazards list, identifying the most significant technology-related risks facing healthcare organizations. For 2026, AI chatbot misuse claimed the number one spot. Not ransomware, not medical device failures, not EHR downtime. AI chatbots used without adequate guardrails in clinical and administrative healthcare settings.
This ranking is significant because ECRI is not an advocacy group or a media outlet looking for clicks. It is an independent, nonprofit organization that has been evaluating healthcare technology safety for over 50 years. When ECRI puts something at the top of their list, it reflects a pattern of real incidents and credible risk that the healthcare industry needs to take seriously.
Why chatbots topped the list
The concern is not that AI chatbots are inherently dangerous. It is that they are being deployed in healthcare settings without the governance structures that clinical tools require. ECRI's report highlights several specific failure modes: chatbots providing medical advice that contradicts clinical guidelines, generating plausible but incorrect information about medications or treatments, and being used by staff as a substitute for clinical decision-making tools without validation.
The speed of adoption is part of the problem. Healthcare organizations are under pressure to implement AI, and chatbot interfaces are the most accessible entry point. A general-purpose chatbot can be deployed on a website in hours. But deploying it is not the same as governing it. Without policies defining what the chatbot can and cannot do, without monitoring for inaccurate outputs, and without human review processes for clinical content, the chatbot becomes an uncontrolled information source operating under the organization's brand and implied authority.
Hallucination risks in clinical settings
Large language models hallucinate. This is not a bug that will be patched in the next version. It is a fundamental characteristic of how these models generate text. They produce statistically probable sequences of words, not verified facts. For most consumer applications, an occasional hallucination is an inconvenience. In a clinical setting, it can be dangerous.
Consider a chatbot deployed by a healthcare organization that is asked about drug interactions. The model may generate a response that sounds authoritative and clinically precise, but contains an error. A staff member who trusts the output because it came from an "official" tool may act on it without independent verification. The more polished and confident the chatbot's language, the less likely a user is to question it.
ECRI's report documents cases where healthcare chatbots provided incorrect dosing guidance, fabricated clinical references, and gave advice that contradicted the organization's own protocols. In each case, the chatbot was deployed without adequate testing against clinical standards and without clear disclaimers about the limitations of AI-generated content.
The governance gap
Healthcare organizations, particularly those in regulated sectors like Ontario's developmental services, have well-established processes for evaluating and deploying clinical technologies. Medical devices go through rigorous testing. EHR systems require validation. New medications face years of clinical trials. But AI chatbots, which can influence clinical decisions and interact directly with patients and families, are often deployed with less scrutiny than a new printer.
The governance gap exists because chatbots do not fit neatly into existing technology assessment frameworks. They are not medical devices in the traditional sense. They are not clinical decision support tools with defined algorithms. They are general-purpose text generators that can be pointed at any topic, including topics where errors have clinical consequences. This ambiguity has allowed chatbots to slip through governance processes that would catch other technologies.
What healthcare organizations should do
ECRI's recommendations center on governance, not avoidance. The message is not to stop using AI. It is to deploy it with the same rigor applied to other clinical technologies. For Canadian healthcare organizations, this means several concrete steps.
First, define the scope. A chatbot that answers questions about visiting hours and parking is fundamentally different from one that answers questions about medications or care plans. The governance requirements should match the risk. Administrative chatbots need basic accuracy monitoring. Anything that touches clinical information needs clinical oversight, validation against guidelines, and clear escalation paths to human experts.
Second, control the training data. A chatbot that is fine-tuned on an organization's own policies, procedures, and clinical guidelines is less likely to hallucinate than a general-purpose model making things up from its training corpus. Self-hosted AI platforms that can be trained on specific, validated content offer a significant advantage here, because the organization controls exactly what the model knows and can restrict what it does not.
Self-hosted vs. cloud: a safety perspective
The self-hosted versus cloud debate is usually framed around privacy and data residency. But ECRI's report adds a safety dimension. When an organization runs its own AI model, it has full control over the model's behaviour. It can restrict the topics the chatbot will discuss. It can enforce guardrails that prevent clinical advice without human review. It can monitor every interaction and flag problematic outputs.
Cloud-hosted chatbots, by contrast, operate as black boxes. The organization sends prompts and receives responses, but has limited ability to control or audit the model's reasoning. When the cloud provider updates the model, as OpenAI and Google do regularly, the chatbot's behaviour can change without the organization's knowledge. A response that was accurate last month may not be accurate after a model update, and the organization has no visibility into what changed.
For healthcare organizations where chatbot accuracy has clinical implications, the ability to control, audit, and version the model is not a luxury. It is a safety requirement. Self-hosted deployment provides this control. Cloud deployment makes it structurally difficult.
ECRI's warning is not that AI should be avoided in healthcare. It is that the current pace of deployment has outstripped the governance frameworks needed to deploy it safely. The organizations that take this seriously, that invest in proper scoping, validation, monitoring, and controlled deployment, will be the ones that realize AI's benefits without creating new categories of risk. The ones that treat chatbot deployment as a quick IT project will eventually find themselves explaining to regulators why an unvalidated AI tool was making clinical claims under their name.
